I want to generate a U-index and then use it as an independent variable to predict general health. What weights?

I want to generate a U-index and then use it as an independent variable to predict general health. What weights should I use? Should I consider it as a two-step prcoess? Specifically, should I first use activity level weights to generate U-index? Then, when predicting general health, should I use individual level weights? I hope my question makes sense. Thanks!
